What Documents and Forms Do You Need to Apply for OAS?

Applying for Old Age Security (OAS) does not mean everyone has to submit the same stack of documents. What you need depends on your residence history, marital status, income information already available to the government and whether another person is handling the application.

For a 2026 OAS application, you need personal and residence information and, where applicable, details about your spouse or common-law partner, recent income and banking information. Paper applicants use Form ISP-3550. Supporting documents may also be required when the information needed to establish your age, identity, legal status or residence is not already available.

Information to Have Ready for an OAS Application

Before starting, gather your OAS application information. You need your residence history since age 18 and the date you want your pension to begin. If you are married or living common-law, you also need your spouse or partner’s Social Insurance Number (SIN) and date of birth.

The 2026 application can also require residence details covering the previous two years for you and your spouse or common-law partner. If you have not filed income tax returns, have foreign income, or recently experienced a reduction in employment or pension income, additional income information may be needed. Banking details are required if you want to set up direct deposit.

This is information you should have available rather than a list of physical documents that every applicant must attach. The documents actually requested can depend on what information is already available and what needs to be established for your application.

Which OAS Application Form Do You Need?

For a paper application in 2026, the main form is the Application for the Old Age Security Pension and the Guaranteed Income Supplement, ISP-3550. The accompanying ISP-3550A reference guide explains how to complete it.

Applicants who meet the online requirements can instead apply through My Service Canada Account. You therefore do not need to download ISP-3550 simply because you need to apply for OAS. Our OAS application guide covers the submission process separately.

For a mailed application, required supporting documents should be provided as certified true copies rather than sending original documents. The paper form also needs to be printed and signed in pen.

Supporting Documents Depend on What Must Be Proven

The 2026 Old Age Security Regulations allow evidence to be required for matters including age and identity, residence and legal residence. That is why there is no single supporting-document checklist that applies identically to every applicant.

Proof of age and identity can include a birth certificate or other acceptable evidence when the government cannot establish those details from information already available. Residence evidence becomes particularly relevant when your Canadian residence history needs verification because years of residence after age 18 affect OAS entitlement.

Applicants who worked outside Canada for a Canadian employer and want that period considered under the applicable OAS residence rules need proof of employment from the employer and proof that they physically returned to Canada, unless they turned 65 while still working abroad and maintained Canadian residence. These are situation-specific documents, not standard attachments for everyone.

Extra Forms When Someone Else Is Involved

Someone who only needs permission to communicate with Service Canada about your application can use the Consent to Communicate Information to an Authorized Person form, ISP-1603. This authorization does not allow that person to apply for benefits on your behalf, change your payment address or change tax withholding.

Different paperwork applies when a person cannot manage their own pension and benefits. A medical professional must complete the Certificate of Incapability OAS/CPP, ISP-3505. A private trustee then uses ISP-3506, while an agency, charitable organization or municipality uses ISP-3507 to establish the appropriate administrative arrangement.

Other Questions About OAS Application Documents

Do you need to provide documents if you apply online?

An online application does not guarantee that no supporting evidence will be needed. The 2026 OAS rules allow additional information or evidence to be requested when necessary to determine entitlement, even after an application has been submitted.

What should you do if you cannot obtain a standard document proving your age?

The OAS Regulations provide alternatives when the normally accepted age and identity records are unavailable. Other information or evidence may be considered, so the practical next step is to contact the OAS program rather than abandoning the application because one record cannot be obtained.

Do you need different paperwork if you apply from outside Canada?

Yes. Someone applying from outside Canada must use a paper application rather than the regular online process. International residence circumstances can also affect what supporting evidence is needed, which we cover separately in applying for OAS outside Canada.

Before sending a paper application, use the document instructions that accompany ISP-3550 to identify the supporting evidence requested for your circumstances. That avoids sending unnecessary personal records while making sure the evidence specifically required for your application is included.
